Generally, passing through rifts into the Fade is possible, but it requires using anchors to stabilize the rift's activity and hold it open—similar to how in the game, disrupting a rift will make it go briefly still and non-crystalline. With some practice, people with anchors may be able to hold the rift stable enough to pass through (it will require at least a couple of them working together, depending on how long it needs to be held open). Rifters and natives with anchors can both do this, and they and anyone with them at the time can all get into the Fade the same way. Rifters will be a little bit brighter in the Fade—not glowing or anything, but sort of like someone's turned up their saturation a few tics, but otherwise it will be the same for them as everyone else. Getting back out would require holding a rift stable in the same way, either from the outside or the inside.
Using rift closures for independent/non-open logs is fine—there are rifts all over Thedas, including creeping steadily northward, other than just those that are used in arrival logs! But keep in mind that it still takes multiple anchors to close a rift.
